Thanks to those who responded to my asking a couple of weeks or so ago if anyone could suggest a model to be followed for an online
form intended to be printed out and posted, incorporating the new gift aid provisions.
Christian Aid were cited by Howard as an example and they have some good online examples and door to door collection wording. Theri online printable form didnt mention gift aid however. Some others suggested the Inland Revenue and other sites to look at which was very useful.
Anyway, I have put a form online which is designed to be printed and it might be a model for other small charities to follow who
can't for whatever reason go in for online credit card donations and need to ask donors to print-and-post. This form is at:

I'd welcome anyone's comments on this, to see if I have actually done a model for others to follow or a disaster to be instantly deleted!!!
